Help with assembly Error

Hello,
I am currently modeling the dynamics of a robot that has a mast that self levels its self in two axis. Currently I am modeling the mast and its nearly complete and looks like it assembles correctly, but I get an error that says it cannot assemble correctly. Looking at the Model Report shows that one of the revolute joints isn't resolving on the rod end of my actuator.
I'm wondering if someone could take a quick look at my model and see if anything sticks out that is wrong. I've made sure all the z axis of the parts line up so i'm not sure what to look for next.
I have attached my model mast.slx and any relevant files.
Please let me know if you have any suggestions.

The Revolute Joint 5 is the source of the problem. Can you clarify what is its purpose? Or is it connected to the proper block?

Hi,
It wasn't easy to figure out what this model is supposed to do, but I did get it to assemble by changing 2 Revolute joints to cylindrical joints so that the frames only needed to be on the same axis, not the exact same location. mast_sm.slx is attached.
