Check the error message on the target console, either by looking at the screen or with the 'getConsoleLog' function: https://www.mathworks.com/help/releases/R2018b/xpc/api/simulinkrealtime.utils.getconsolelog.html?s_tid=doc_ta
It will provide useful data about what the actual cause of the failure is.

If the error in the console is:
"Unable to load DLL dependency: KERNEL32.dll.GetFileSizeEx"
then you are probably affected by the bug described here:
But there can be possible other causes. Please contact MathWorks Technical Support for help: https://www.mathworks.com/support/contact_us.html

I also had this issue when using Simulink Realtime R2017b - my error included the message that
The call to slrt_make_rtw_hook, during the exit hook generated the following error: Invalid Simulink object name: loader
My fix was found by clearing more space on my target PC.
