frequency modulation sinwave with 3 different frequencies

Hi, i want to make a sinwave that will change his frequency 3 times (like in the picture i uploaded
), i tryied with fmmod function but i can't make it work. So is there any other way to do it? Note f1=40Hz , f2=50 and f3=80 fs =100 thanks in advance.
