I would like to import a csv file to my fis

I have a csv file containing all of combinations of fuzzy rules and their outputs. I would like to import this file to my fuzzy inference system to view the surface plot and firing of rules, is there any way to do this?

Csv file contains 4 columns and 125 rows. Columns 1 - 3 represent my input values each with 5 membership functions (5*5*5 =125 rules) and column 4 represents my output which has 5 membership functions. These are all in linguistic form i.e if "severity is low and occurane is high...then"
