Creating binary intracranial mask

Hello all,
I would like to remove the skin voxels and keep only the intracranial brain regions in the 3D image volume(a slice shown here)
All the white voxels are 1 and it's a binary mask. How do I do that?

Hi, I specified. So the intracranial brain and the skin voxels has similar intensity. The binary mask was generated from the intensity thresholding. CT image has this property in head.
I checked some functions like imopen which kind of remove the skin outside, but not good enough.

I mean can you specify them in image, which is intracranial brain and which is skin voxels??
